Card Activation

| Call the phone number on the activation sticker on the front of your card (1-877-878-0435). Follow the voice prompts. |
| To activate your Card you will need the last 4 digits of your social security number. If you do not have a social security number, then use the last 4 digits of your primary identification number, such as federal tax ID, green card number, work or student visa number or another personal identification number that you used to subscribe to the Family Matters Card. |
| When you activate your Card, you will be prompted to choose a Personal Identification Number, or PIN. This PIN is an important security feature of the program. The PIN will be needed when you use an ATM (automated teller machine) and to access account information over the internet or by phone. |
| Choose a PIN that is easy for you to remember but that others are likely not to guess. Do not lose or forget your PIN but also do not keep you PIN written down on your Card, and do not keep it in the same place as your card. |
